Tesamorelin is a synthetic analogue of GHRH, growth hormone-releasing hormone. It consists of the first 44 amino acids of human GHRH with a trans-3-hexenoyl group added at the N-terminus. That modification makes the molecule resistant to degradation by DPP-4, the enzyme that inactivates natural GHRH within minutes.
Its principle therefore differs fundamentally from administering growth hormone itself: tesamorelin acts on the GHRH receptor in the pituitary and stimulates the body's own pulsatile release of growth hormone rather than replacing it. The body's feedback loop stays intact.
